If you have a question or issue
with an ExxonMobil benefits service provider (ExxonMobil Benefits Service
Center, CitiStreet, Aetna, Medco, etc.) that you are unable to resolve with the
service provider you can contact the ExxonMobil Benefits office by completing an
e-mail form.

You will receive an acknowledgement by mail,
telephone or e-mail within 1-2 business days and a response as soon as possible.
Reporting Retiree or Surviving Spouse Death
To report the death of an ExxonMobil, Exxon, Mobil or Superior Oil retiree or
surviving spouse, write or call the ExxonMobil Benefits Service
Center at 1-800-682-2847
The caller should provide the following
information:
- Deceased's Social Security number
- Date of death,
- Name, address and telephone number of the spouse or other survivor. Whenever
possible, the call should be made within one week of the death.
Upon notification of the death, the EMBSC will
send a Family Status Affidavit to the surviving
spouse/beneficiary. When the affidavit is received by the EMBSC, along with an
original death certificate, the process for the payment of any retiree death
benefits begins.
Other information that will
be needed:
- Marriage certificate and, if married, spouse's birth certificate
- Social security numbers of both retiree and spouse
- If retiree and/or spouse were married previously, copy of divorce decree or
spouse's death certificate, whichever terminated the marriage(s)
- Instructions to notify the EMBSC as soon as possible following the retiree's
death
